T-minus 48 hours
Ask the floor RN or case manager when discharge is planned. Get the discharge summary, the new prescription list, and the follow-up appointment list emailed or printed. T-minus 24 hours
Pick the room your loved one will spend the first 72 hours in (often the bedroom or den). Move what they need within arm-reach: water, glasses, phone charger, the new medication bottles, the call bell or baby monitor if mobility is reduced. Clear throw rugs and add a nightlight to the bathroom path.
Discharge morning

First 24 hours home
